Your application will not be approved until we receive your business identification. Please fax to 905-670-3347 or scan and email to info@stargazeroriginals.com two ofthe following;

  • Retail Sales Tax / Vendors Permit
  • Recent Invoice from similar industry supplier
  • Yellow Page Ad or Other Promotional Ad

If proof of business is not received with 48 hours, your application will be deleted.

Current customers with recent account activity do not need to submit businessidentification.

Once your application has been processed, an approval email will be sent to theemail address listed on form. If you do not receive, please check your spam box.
